The DHHS Transportation Portal is a tool to manage your Family and Friends trips. You can review, request, and cancel your trips. It also includes an important Family and Friends trip completion verification feature, which, if used, will help expedite the reimbursement process.

FAMILY AND FRIENDS FREQUENTLY ASKED QUESTIONS AND FORMS

This guide answers some of the most common questions people have about the Family and Friends Program.

It is required for you to complete the “Enrollment Form” for any and all drivers you use for Family and Friends. We recommend that you pro-actively provide CTS with this completed form along with copies of driver licenses for all drivers that you use – including yourself.

It is required for a mobility form to be completed by a medical provider or Care Manager and sent to CTS prior to the appointment date for specialized modes of transportation, such as wheelchair, stretcher van, or non-emergency ambulance.

Reimbursement for transportation to Medicaid-covered services will only be allowed if you call CTS prior to your appointment so CTS can pre-book the trip and verify that transportation will be covered.

This form must be completed in its entirety to receive reimbursement. However, you can download it for future use.

Forms must be submitted within 60 calendar days from the last date of service on the form.

  • Email – a scanned or legible photograph from a smartphone to FF@ctstransit.com .
  • Fax – to (203) 375-0516.

MEMBER TRANSPORTATION PORTAL/APP HOW TO GUIDE

NOTE:  If you are having trouble registering / or are locked out of the Portal, it can be because DHHS does not have your email, or current email, as part of your contact information.  Please ensure that the email address you use for registration matches the one on file with DHHS. If you need to update your email address or any other contact information in the future, you must do so directly with the State by contacting DHHS at 844-275-3447.

Please note that updates may take 24-48 hours for CTS to receive the update. You will have to re-register with a new account in the Portal using your new email address.

In this guide, you will learn how to look at your trips in the portal. This includes trips for both Family and Friends rides and any ride with a traditional transportation provider on a specific day, future trips, trip details, trip statuses, and repeating trips.

This guide will teach you how to book your trips and manage your drivers.

CTS cannot reimburse your trips without receiving a timely, correctly filled, legible and signed reimbursement form AND additional confirmation that you attended your medical visit. The quickest way to provide this additional confirmation is to use the portal’s “trip verification” feature. In this guide, you will learn how to confirm the completion of your trips using this feature.
